Registered Nurse / LPN

Massachusetts · Field-based · Full time and per diem

Visit members at home, review medication and health status, write and update clinical documentation, and coordinate with prescribers and case managers. You carry a caseload rather than a shift.

What we ask for

Active Massachusetts RN or LPN license in good standing
Comfort working independently in members' homes
Reliable transportation and a valid driver's license
A second language is valued

Case Manager

Massachusetts · Field-based · Full time and per diem

Own a caseload of members and their caregivers. Complete home visits, build and maintain care plans, support caregivers through training and paperwork, and keep enrollment and documentation current with MassHealth and the health plans.

What we ask for

Bachelor's degree in social work, human services, psychology or a related field
Experience in case management, home care or community health
Reliable transportation and a valid driver's license
A second language is valued
