Massachusetts Adult Foster Care Program (AFC)

Adult Foster Care (AFC) is a MassHealth program that helps eligible members remain safely at home with the support of a live-in caregiver. Caregivers receive training, ongoing support from registered nurses and a care-management team, and a monthly tax-free stipend.

Eligibility Requirements for Caregivers and Care Recipients

You may qualify for the Massachusetts Adult Foster Care (AFC) program if the following applies to you and your loved one. Final eligibility is determined following a care assessment and physician authorization.

Care Recipient
Lives in Massachusetts
Is at least 16 years old
Is enrolled in a qualifying MassHealth plan - MassHealth Standard or CommonHealth, or an integrated plan such as One Care, Senior Care Options (SCO)
Needs regular help or supervision with at least one activity of daily living - such as bathing, dressing, eating, using the toilet, or moving around
Caregiver
Is 18 years or older
Lives in the same home as the person they care for
Is not the spouse of, or a person legally responsible for, the care recipient (for example, a parent/guardian of a minor member)
Can commit to ongoing caregiving responsibilities

Can I receive a stipend for caring for my parent or another family member?

If you and your loved one meet the Massachusetts AFC requirements, you may be eligible for a monthly caregiver stipend. Spouses and people legally responsible for the care recipient are not eligible to serve as the paid caregiver.
